This person will be contacted to verify (or modify) the appointment. Public prayer request that will be placed in the Church program's Circle of Concern (requesting others to pray)	Name of person(s) prayer is requested for and short description of the circumstances. Request to be Baptised	Name of person requesting to be baptised and how the church can contact them (phone number, e-mail address, mailing address). This person's name will be placed on a list and they will be contacted when the next baptism ceremony is scheduled. Food, shelter, clothing	None	Contact the Church office.
Request to become a member of the church	Name of person(s) who wishes to become a member and how the church can contact them (phone number, e-mail address, mailing address). This person's name will be placed on a list and they will be contacted when the next membership class is scheduled.
