How do I Enroll?

Preschool Registration/Enrollment

The initial requirement for registration is a meeting between the parents and a qualified staff member to determine whether the school's program is appropriate for the child.  Usually a tour of the facilities is done at the time of this meeting.  Once the decision is made to enroll the child, the parents are asked to fill out a set of enrollment papers, most of which are required by the state.

The following forms are required:

  • Registration Agreement
  • Family Information and Health History,
  • Physician's Statement,
  • Identification and Emergency,
  • Parent's Rights,
  • Personal Fights,
  • Additional Information Form, and
  • Emergency cards.

It is the parent's responsibility to inform the school immediately of any changes in emergency information.  Please notify us of new addresses, home & work phone numbers.  Changes in your authorization to pick up your child list and any changes in health such as newly diagnosed conditions, treatments, allergies, etc.

In addition, proof that all state required immunizations are up-to-date is required.   The immunizations required for admission to preschool are: 4 DPT, 3 Polio, 1 MMR, 1 HIB, 3 HepB, and a current TB test.

A non-refundable registration fee (see fee schedule) is required at the time of registration and at the beginning of each fiscal year (July 1).  Once the initial registration fee is paid, a place is reserved for that child for that year.


Insurance

Your child is insured in the event that he/she is injured while attending preschool or participating in related activities. However, it is a secondary insurance to any policy that the parents may have and the parents are responsible for paying the modest deductible fee.


Withdrawal

Please notify the director in writing at least one week in advance before withdrawing a child from the program.  Withdrawal without notice is subject to closing charge of one week's tuition.
